Short Term Rehabilitation
The staff at Springwood Healthcare Center are here to make the transition from the hospital to post- acute care easy. Patients who are admitted for short term rehabilitation have experienced an injury or illness that is expected to improve with the intervention of therapy and nursing supervision and return to their prior living setting.
Each member of our interdisciplinary team does an evaluation of the patient at admission that looks at their prior level of functioning, their medical history, their current level of functioning and plans are for a safe discharge. Our short term patients are evaluated for the amount of physical and occupational therapy they are able to tolerate. Speech Therapy also evaluates a patient if indicated.
Nursing provides medical monitoring of the patient’s medical condition, administers medications. Our wound care nurses focus on dressing changes to a patients’ post-surgical site and any skin concerns. Certified nursing assistants support our registered nurses with monitoring of vital signs and medical conditions and provide help to patients with bathing, dressing, eating and transferring. Together, our team supports the patient to meet their goal of being as independent, to celebrate success every step of the way.
